Dr. Santan currently sees patients between the hours of:

Monday-Friday	9:00 am - 5:00 pm ET
Saturdays	10:00 am - 2:00 pm ET*

*An 'after hours fee' applies. Saturdays are telehealth only at this time.

Step 1: Fill out the Appointment Request Form Below

Click Here and select the appropriate appointment request form. This is information we need to set up an account for you in the portal.

Step 2: Complete Your Portal Profile and New Patient Paperwork

Once we wet up an account for you in the portal, you'll receive instructions for logging in and completing your portal profile and new patient paperwork.

Step 3: Schedule Your Appointment From Within the Portal

Once your portal profile and new patient paperwork is complete, you'll get access to the scheduling calendar from within the portal to schedule appointments.
